Information you choose to provide
The consultation form may collect your name, email address, optional phone number, service interest, and message. The future-services interest form may collect your name, email address, state or jurisdiction, area of interest, and optional notes.
Please do not submit urgent, confidential, or detailed health information through these forms. Submitting a form does not create a therapist-client, psychologist-client, or other clinical relationship.
How information is used
Information is used to respond to inquiries, arrange introductory conversations, understand interest in current or possible future services, maintain appropriate business records, and protect the website from misuse.
How information is handled
Form submissions are stored in the website’s database. Consultation requests are also delivered to an Actualization& Google Workspace inbox so that Christian Ishak can respond. Website hosting, form-processing, database, and email providers may process the information as necessary to provide those services.
Information is retained only as long as reasonably necessary for the purposes described here, for legitimate recordkeeping, or as required by law. It is not sold. It may be disclosed if required by law or when reasonably necessary to protect rights, safety, or the integrity of the website.
Technical information
Like most websites, hosting systems may process limited technical information such as IP address, browser type, requested pages, and timestamps for security, reliability, and troubleshooting.
Google Analytics may be used to understand aggregate website use, such as which pages are visited and whether visitors complete an inquiry form. Google may process device, browser, approximate location, and interaction information and may use cookies or similar technologies. Actualization& does not use this information for targeted advertising. You can limit cookies through your browser settings or use Google’s Analytics opt-out tools.
Your choices
